Personal Service Care Plan

The process involves these steps:

• Telephone call or email to Health Care Plus, LLC
• Health Care Plus's Clinical Director or assigned Registered Nurse Supervisor provides the initial assessment at the Client home
• A Service Plan of Care is produced with collaborative efforts of Health Care Plus's Clinical Director or RN Supervisor, the Client, and Family Members.
